ILR - absence letter queries and format

Hi,
I am a Tier 1 VISA holder applying for ILR in May this year.
As per Section 6.2 in ILR SET (O) form regarding immigration history, we need to list any absences in the UK however short with reason for absence. Also, evidence of all work-related absences (including paid annual leave) is required.
I have worked as permanent employee for 3 years (in 2 different companies) and currently working as a limited company director (close to 2 years).
When working as a permanent Employee:
1. I had been to India thrice each for around 15-20 working days to meet my family and friends. All of them were holidays taken from my annual leave entitlement. Should I mention this as reason for absences?
2. I had visited few other countries for entertainment and leisure. All holidays were taken from my annual leave entitlement. What reason should I mention?
Do I need to provide evidence letter for above leaves? If yes, what details need to be included in the evidence letter for these holidays. What details should I ask my employers to include in the absence letter? Just leaves taken from so and so dates and my position/salary during that period? anything else?
Basically, I am looking for a format of letter that I need to ask my previous employers to provide? Should it be addressed to UKBA?

1. I had been to India couple of times each close to 20 working days to visit my family and friends. I understand that I need to provide a letter regarding these absences. What details go in that and what should be the reason for absence? Is there any specific format that can be followed or referred?
