5y Tier 2 General To ILR

Hi,
I am eligible to appy for ILR by sep 30 2014, which is 28 days before i complete 5 y in UK.
My query:
I have seen an opportunity for another job, but the new employer hasnt had any sponsorship license before and would like to go thru the process. But I dont know whether it is advisible to change now, as I am close to getting a ILR in september 2014.
Does it affect me in any way if I change a job in last six months of tier 2 general visa, before ILR?
Also
Tier 2 applicant + a PBS dependent who can apply on the same SETO form, how much do they pay for a premium day service, is it 2193+400+400 = 2993?? or is it just 2593.
As I couldnt find this information on the new application form.
