ILR Query- Tier1 Dependent and Director of a Limited Company

Hi,
I am currently in UK as my wife's dependent on Tier1. My wife has been in full time permanent employment in last 5 years. We both came to UK in May 2009 on Tier 2 ICT.
She converted her Tier 2 ICT to Tier 1 General in Feb 2010 and I had to go out of country for a week and I came back as Tier 1 General - Dependent. I continued to work on this dependent VISA for some time and started my own limited company in Nov 2011.
We will be applying for ILR together in couple of weeks.
My question is -
- Since I am currently on Tier 1 dependent, I will be applying as dependent.
Do I need to submit documents of my limited company at all?
- In my situation, do we fill seperate SET-O forms or one single form for both of us is sufficient.
- We got our last extension in Jan 2013. Is it OK if we show cohabitation proof from that point onward or it has be last 5 years (or 2 years..not sure what exactly is the requirement)
- My wife has changed jobs in last 5 year. I have all the P60s but can't find her P45. Do we need to show that?
