Question Regarding SET(O) form Section 5

Hi Guys,

I am on Tier-1 General and my first Tier-1 stamp was on 7-July-2009 , got extension in 2012 and now my visa is expiring on 7th July 2014. My daughter was born on 10th May 2011.

Me and my spouse ILR PEO appointment is next week so i need help regarding SET(O) section 5 form,

Section5.7
1) Please give the start and end dates of the period of maternity, paternity or adoption-related
absence (if you are still absent from the workplace due to maternity or adoption-related leave,
the end date should be left blank).

I got only one week parental leave but i dont have any letter or proof right now? what to do ?
