ILR Dependant

Hi,
I am currently on ILR and my wife is on my dependent visa since 2.5 years. Her visa is expiring in Aug-2014.
I would like to apply for her extension and as she has completed her 2 years now, so I guess we can apply for her ILR visa as well.
My question is, which form do I really need to fill for her? Is it Set (O) or some other form? Also, I remember while taking my ILR visa, there was a rule that I shouldn't have spent more than 180 days out of UK in 5 years. Is there any such rule for ILR dependant visa as well?
Can all the gurus here please help me to find the right application form and sort out my query?
