New SetO form help

Hi,
I am on T1G visa and going to apply ILR. I am filling up the new SetO form version 06/14 and have following doubts, hope you guys will be able to help with it:

1. Page 27 question 7.11 how long have you lived in the uk? I am in the uk since 2005 but my initial t1g visa was granted in 2009 so should I mention the total period from 2005 or from 2009?
2. Page 57 question 10.6 place of issue of BRP? I checked my BRP but could not find the “place of issue”. My last t1g PEO appointment was at Solihull in 2012. Not sure where to find the place of issue.

You should calculate from 2009 as you are applying under Tier-1

it says UK usually on all of them
