the 28 days rule - confused when I can apply for ILR

Gurus/experienced members please help.

1) I got my tier 2 general visa in country issued date on 15 June 2010. I was already in the UK during that whole time (on student visa).
2) My first day physically at work was 15 July 2010.
3) I then travel out of the country for a new hire trip and back in the UK, passport stamp in 25th July 2010.

If I'm applying the 28 days rule, which date am I eligible to make my ILR application base on 5 years tier 2 G please?
I'm using the date in number 1 as when the clock of 5 years continuous started. Is it correct? So I will be able to make my application on 20th May 2015? Can someone please confirm? Or I have to use the first date as number 2 (I don't think it should be number 3 but just checking).

Second question, I lost my old biometric card after my tier 2 was extended in 2013 (either that or they never returned it back to me, I just cant remember). Is old biometric card one of the document you have to submit for ILR application, and if not, how do you approve that you have completed 5 years (that your first tier 2 visa was issued on date XYZ?

Please help me with my questions, as it's getting closer to the date, I'm just getting super anxious and worried all the time.

28 days before 15th June if this is the date your visa started.

Did you apply for a replacement BRP?

BRP and all the other documents are required when applying for ILR. Have you looked at the form and guidance notes?

Thanks CR001, I thought so regarding the date.

As for the biometric card. I applied for tier 2 extension in 2013, so I have my new card. But I couldn't find my old card (the one from 2010 to 2013), and I'm not sure if I lost it or because UKBA kept it when I applied for the extension and didn't return it back to me. Do you/anyone know what's the protocol here when applying for extension?

And for ILR application, do you need to provide ALL of your current and past biometric cards, or just the current? And if not the old one, how do they know you have completed your 5 years on exactly which date (ie how do they know my old card started on 15 June 2010).

just the current
how do they know you have completed your 5 years on exactly which date
They have your full immigration on their system so will know what your visa dates and extension dates are etc.
