10 years Long residency on Premium Service

Hello,
I have recently booked a Premium service appointment for SET(LR) at a PEO.
I did not have to fill in an online form for this appointment. Does this mean that I should fill in the latest SET(LR) form by hand and take to the appointment?
In the SET(LR) form, there is no mention of the Premium service. The only payment option mentioned is £1500 for postal application.
I would be grateful if anyone who has done this could reply.
