ILR application with wife

Can someone please clarify whether I need to submit a separate application form for my wife or do I include her within my SET O form
I'm currently on a Tier 1 general since Dec 2010 and my wife has been in the UK since Sep 2011

Wife and I have an UK degree. In that case I believe 4.5, 4.6, 4.7 and 4.8 are not applicable
Can someone please confirm
4.5 If you have a speaking and listening qualification in English at B1 CEFR or higher, or an equivalent level qualification, is it:
A test that is on the Home Office list of approved tests – go to question 4.6
A test that is on the Ofqual register or awarded by the Scottish Qualifications Authority – go to section 5
4.6 When did you sit your English language test? On or after 6 April 2015 – go to question 4.7. Before 6 April 2015 – go to question 4.8.

Not applicable to you if you have a UK degree.
