When is a postal application counted as "in-time"?!

This is something I am a little confused about. Does the "in-time" terminology refer to when the application was posted or when it was received by the Home Office?
E.g-
1. Would an application be considered "in-time" if it was posted on the last day of the visa which reaches the home office the next day?
2. Would an application be considered "in-time" if it was posted a couple of days earlier than the current visa expiry date, but then it was a wrong form so had to be changed after the visa has expired.
3. Would an application be considered "in-time" if it was posted on the last day of the visa which reaches the home office the next day but then the applicant wants to either vary or upgrade to premium service?
