ILR for family

Hello,
Just wanted to know about the filling of the SET O form. My wife, son (5 years) and myself are all on Tier 2 and will be completing 5 years this september. Had a look at the SET O forms.
Do we fill 3 SET O forms when submitting? Because the SET O form states that main and 2 dependents. But I do not want my wife to be an ILR dependent rather just ILR.
We have another son who is born in the UK but has an indian passport. Once we get our ILR do we go through the naturalisation process for him?
