ILR for Tier II (General) Dependant

Hi

I was granted a Tier II (General) visa in Oct 2012. I stayed on that visa for 5 years (including renewal), and was recently granted the ILR in Oct 2017.

My wife joined me as a Tier II (General) Dependant in Feb 2013. She will be eligible for ILR in Feb 2018. My questions are

1. What form do I need to fill for her ILR application? Is it the Set (O) or Set (M) or something else?
2. How long do I have to prove cohabitation for? Is it 2 years or 5 years?
3. My wife is also a salaried professional. Does she need to provide her payslips, P60 and letter from her employer supporting her ILR application?
4. Do I, the ILR holder, need to submit my payslips, letter from employer and P60?
5. What documents can be used to prove cohabitation?
6. Do I need to provide my marriage certificate?
7. Do I need to provide my bank statements?
8. What is the earliest date I can apply for her appointment? Is it 28 days before her 5 year anniversary of entering the UK? Or can she apply only after the 5 years have fully completed?

1. Set(O) if she holds a PBS Dependent visa.

2. For 2 years.

3. No

4. Not sure.

5. Tenancy agreement, letters from the council, utility bills etc etc.

6. Yes

7. Probably, not sure.

8. No sooner than within the 28 days before the 5 year anniversary of the date she entered the UK.

Thanks for your reply. Here are some additional questions

1. Can a bank statement (which mentions both of us as the account holders) be provided as proof of cohabitation

2. While filling the financial details part of the form, do I (the ILR holder) need to provide my salary along with SOC code? Or should this be left vacant as I have already received the ILR and am only applying for my wife.

3. Do I (the ILR holder) also need to provide passport sized photos?
