ILR AND PBS DEPENDENT APPLICATION

Hi,
I will soon apply for ILR. My family is yet not eligible for the same and would be applying for PBS dependent extension. We will be applying via premium service.
My query was with regards to what form I would need to fill up for my family members?- would this be an online application or a downloadable form?
Also will my wife ( and me) require a criminal records certificate?
