I have filled 4.3 with yes to Life in UK test and academic qualification by UK NARIC master's degree.

Can I skip 4.4 and 4.5, the document is not clear.

Any help is appreciated.

4.4 is talking about exemptions so you can ignore that as long as you have ticked "UK NARIC has confirmed that the qualification was taught or researched in English to the appropriate level " in the previous step

4.5 is only applicable if you took SELT test

so given the above I believe you can leave those. If you have a dependant please consider that as it is revelent for both so for example you maybe giving UK NARIC but your dependant maybe taking SELT
