ILR - Premium service booking clarification

Dear Experts,
I am completing 5 years on 3rd august 2018 (tier 2 route), To start my ILR process, I booked premium service on 9th july. Hope members here could calrify few doubts with my ILR application.
1.) After my premium appoinment booking, I see in document checklist, under "what happens next" section, it says
"If you have not already done so, you will need to download and print your application form by signing in to your booking.Complete and sign the application form and bring it to your appointment."
I downloaded the application form. Its only two pages. Wondering if I done something wrong?
Also in the application form, the date of application is mentioned as 29th may. Is this going to be issue
when I appear for premium service on 9th July.
2.) Is the following list of documents enough for submission.
Appointment Booking Document Checklist
Completed SET (O) Application Form
Life in the UK Test PASS certificate
Master’s degree certificate – (Taught in UK)
Letter from employer confirming the annual salary and continued employment
Letter from employer confirming absences from UK in the last 5 years
Last 3-month Payslips
Personal bank statements for the past 3 months
Identical photographs with full name on the back
Biometric residence permit
Current Passport & Old Passport

I think you only have reserved your PSC booking ... SET (o) form is 86 pages , you have to print , complete it and take it to your appointment unless you have completed online form which i am not sure how it works ....

It is 86 pages, in regards to the PSC, as you have already booked and paid online, you do not need the payment page of the SET(O) form, just your booking details and payment page when you booked.

I am also applying ilr on tier 2 route and booked premium service on 10th july.
Can you pl clarify this thing for me
After my booking on my confirmation printout it say
visa-SET (O) - Settlement – other
and on
Appointment Booking Checklist it says
Applicant
Product name/service: SET (O) - Settlement – other
I am confused as I filled as settlement - 'Tier 2' not 'other'
Can you pl clarify whether it is ok or i have done some mistake and have to call someone.

I filled the appointment booking as Tier 2 but in the document checklist, the product name is mentioned as "SET(o) Settlement Other". I guess its ok.

Do you not need need to provide utility bills or P60 for past 5 years to prove residence in the UK during the whole time?
Not trying to confuse you, asking for myself as I am applying soon and thought that was also in the list of docs for this route.

Dear Raja,
I am in exactly the same situation as you and i am deeply confused by this new online ilr application system. Have you found out whether it is valid to submit the form online and book a PSC appointment as long as the appointment date follows the 28day rule. Could you pls let me know? Thank you!!!

It’s is best to fill paper form and take to PSC appointment and PSC appointment day will be considered date of application...,
If application form filled online and submitted then date of application will be the date you submitted your application online and not PSC appointment day....somebody could get refusal of application by applying too early before completing qualifying period...
