set m and o maternity

Hi, my brp expires soon and I am just finishing my set m forms but have a few questions.
1)i am currently on maternity leave took normal 9 months and now taking 3 months unpaid, i am normally on salaried employment so when filling out my income shall i fill out my normal pay or shall i put 0 as not earning atm but returning to work in august. my income before maternity met the income threshold but just in case i am putting my husbands details too. regarding my payslips shall i just put my last 6 months which will show statutory maternity pay and then zero 0 for 3 months extra or??
2)my husband is self employed so i am putting his income too. what documents do i need and will this be a problem
3) My baby was born in Uk and is 10 months what documents can i provide for her as they need be in the past 3 months
