Tier 2 to ILR - Questions help

Hello Admin and members,
I am applying for ILR using Set (O) form based on Tier 2 route. There are few questions in the form I am confused about. Please help:
1. ‘Documents you providing as evidence for absence from the UK:
For this, is the letter from the employer confirming when I took the annual leave sufficient? Or do I have to provide any other document?
2. Have you previously lived in a country outside the UK including your country of birth?
Do I have to answer yes to this question? I was born in India but moved to UK in 2008. I have never lived in any other country except India and UK
3. Have you ever been refused a visa for UK?
My visa was rejected once in 2011. I was applying for Tier 1 (General) and was refused on the basis of not enough income. Do I have to include that?
4. I am applying with my Partner. There is a question regarding if visa was granted before Jan 2018 immigration rules. Our last extension was granted in September 2016. So do I have to say yes to this question?

Are you sure you are using the correct set O form? I cant find your questions 2 and 3 in the set o form

Are you sure you are using the correct set O form? I cant find your questions 2 and 3 in the set o form
Thanks for replying. I think I am using Set (O) form. Its the online version I am filling to book the appointment. All these questions are available online. I will check again though.

That's fine. It is possible they are in the online form and not in the paper form
