HI there,
I have my premium appointment for SET (M) tomorrow. I have downloaded and filled in the SET(M) form version 04/2018, but when I log on to my booking online it asks me to download my application form on there. When I download this link it doesn't come up with the same application form, just a 2 page summary of the applicants details (personal information, payment).

Have I filled in the correct form, or is there a different one I need for a premium appointment?

Spouse visa on FLR(M) or FLR(FP)??

My previous visa 2.5years ago was FLR(M)

Set(M) is the correct form for ILR.

thank you - this is the form I have printed an filled out. However, my confusion is on the online booking where it tells me to download the link to the application form, but this link is only a 2 page summary.
I wondered if I had to take a different form into a premium service, or is my SET(M) that I downloaded previously fine.

Your downloaded Set(M) paper is the correct one.

The summary is just what you booked etc and you can carry it with you too.

Good luck from me too! I have seen other references to the two page form you are being directed to. I would also fill that out if I were you, just in case you need that one as well as the long SET(M) form.
