FLR (M) financial requirement section: category F: information

Hello
Need some help as i'm filling the FLR-M and wanted to be 100% sure even though some of this is self explanatory,
Category F (limited company reg. in UK - last full financial year)
1) The income needs to be total salary and dividends received during the tax year of company as specified on the Ct600. For my company, the year end is 31-may.
So in the income from self employment Box on page 25 of the form,
I need to add the dividends + salary received from 1-jun-17 to 31-may-18?
2)page 27 (section 8.3B)
option (iv): address of the company: since my co. is regd. to my accountant's address, I take it I need to use this, as this appears on all the accounts related pages?
3)page 27 (section 8.3B)
are you/your sponsor registered as self employed in the UK with HMRC?
since i'm the sole director and receive salary and dividends, I take it I need to say YES? I don't have any explicit documentation answering the above question but i do file the self assessments every year.
