Applying ILR (combining tier1E and tier2 G)

Hello and good morning!
My timeline is as follow:
I got tier1 entrepreneur visa in 50K category, on 9th January 2014. I converted this into tier 2 general visa on 23rd September 2016. During my stay on tier 1 entrepreneur, I created two jobs, paid taxes and everything related. The only thing, which I could not do unfortunately, was the investment of 50K, only managed to invest a sum of 42K. The reason for not investing in full was that I changed my tier 1 entrepreneur to tier2 general visa before the expiry of tier 1 entrepreneur visa’s tenure.
Currently, I am on tier 2 general visa since 23rd September 2016, earning the required salary.
I believe I will be eligible for indefinite leave to remain on 9th January 2019 on the basis of spending 5 years lawfully (time spent on tier1 entrepreneur plus time spent on tier2 general).
My questions are:
1) What documents do I need to provide regarding my tier 1 entrepreneur visa?
2) Not spending the full 50K, will it affect my application?
3) I cant see any section of the SET(O) where I can mention that I am applying on the basis of “combining route”, what and where to mention on the form?
4) I am planning to apply on the premium service in second week of January 2019; someone told me that they are finishing the premier service! Is this true?

No documents

No

There is no such route. You should apply under Tier 2 general and be eligible for ILR under that route. They will consider the time spent under Tier 1E towards the 5 years, that is all

I have got my CoS letter which is showing a salary of 22k because at that time I was a Trainee Pharmacist.
Now, I have become a full Pharmacist and my salary is above the threshold for the ILR, but it is not apparent on the CoS letter.
My salary slips and bank statements show that I am getting the salary required for the ILR.
I am just wondering will this affect my case? please!

There is NO requirement to submit a 'cos letter' for ILR.
Have all your salary increases and promotions been updated in the SMS system?

No, it has not been updated as I am not aware of that. Could you please let me know how to do that please!

Your sponsor has an obligation under their sponsorship duties to update the SMS system when only your salary etc changes (not company wide annual increases). You cannot update the SMS system, only the Level 1 users/company employee can.
If your SOC and job changed, you would have probably needed a new CoS.

Thanks for your kind words.
I am the only worker working for the company. My SoC has not changed, but my title has changed from
pre-registration pharmacist to Pharmacist.
Could I ask my employer to make the necessary changes in the SMS? If yes, how long does it take to appear in the SMS? Please!

just to explain it a bit more.
I am only getting the required threshold of the salary from August 2018, as I only became pharmacist in August 2018. Before that, I was employed by the same company from 2016, but was getting only 22K as a trainee pharmacist.
After passing the exam in August 2018, I am getting the required amount for the ILR. I am planning to apply in February 2019, till that time I would have worked for 6 months as a pharmacist. It means that I would have only 6 pay slips touching the salary threshold. All my previous P60s only show an annual salary of 22K.
My employer has not updated my new salary (although my SoC code has not changed) on the SMS.
What should be the best course of action for me? Please! I am really concern!

My SOC has not changed but my job title has changed from "pre registration pharmacist to pharmacist". Could I ask my employer to mention this in the sponsor note section of the sponsorship management system (SMS) please?
Will that be enough to clear any doubt(s)? please!
