ILR for PBS dependant minor

Hello,

I am applying ILR for my 7 year child using SET (O) form under PBS dependant category.
Me and my husband are on ILR, My Husband is main applicant and I got my ILR as his dependant. while submitting application for my daughter, I am confused about below questions:


1)In document section of form- "I will provide a declaration signed by me to confirm my consent for the Home Office to request verification checks" - Who need to sign this ?
2)In declaration section of form what option I need to select
"I am the applicant"
OR
"I am a representative of the applicant and have been authorised to act on their behalf and have discussed and confirmed the contents of this application with them"

3) what supporting documents I need to submit apart from my daughter BRP and passport.

All signatures will be of the parent/guardian, use 'I am the applicant'.
You need to provide copies of the parents ILR too

one more query do I need to get bank statement of my husband as there were questions on form:

"Does the account with the bank or utility company for which you have supplied information and supporting documentation relate to another person who is to provide you financial support?"

"I will provide a declaration signed by each of the customers on an account with a bank or utility company relating to another person who is to provide me with financial support"

Also do I need to get letter from GP or her school? birth certificate Etc?
