Right Form for ILR as BPS Dependent

Hi,
I was on Tier 2 and then moved to ILR and then got my UK citizenship in the beginning of 2018.
When I moved to ILR, my wife was in UK for 3 years until then, hence I had to get her visa extended under same category of Tier 2 dependent.
She is now eligible to apply for ILR, as she has completed her 5 years in UK.
Just to make sure that I don't do any mistake and end up wasting my fee :), I should be using Set (O) form online for her ILR visa and apply online? Is that correct?

Also, I am providing the following documents:
1. Her passports and BRP
2. Her UK Degree (to cover English Language)
3. She has done her Life in UK already, so providing the ID on the Set(O) form.
4. Copy of my British passport (optional, as I have already provided my passport details in her Set(O) form.
5. My payslips
6. My 6 months bank statement
7. Two utility bills with both of our names on it (British Gas and Council Tax).
8. Consent forms signed by myself and my wife.

Am I missing anything?

Yes Set O is correct if she has held a PBS dependent visa for 5 years

Not required. There is no financial requirement for PBS dependents applying for ilr.
7. Two utility bills with both of our names on it (British Gas and Council Tax).
You need more than that. You need 3 to 6 sources in both names spread evenly over the last two years.
