Form FLR M

I'm completing form FLR M online, for my daughter in law who is a Thai national.

There is no guidance for completing the questions on the form and the Home Office adviser could nothelp me. Can anyone help with the following questions and how they should be answered please.

Other Name
I have entered her birth name here, but am unsure of the question that asks "Have you legally changed your name and when?" Does this refer to when she married and changed her name to my son's? There is nowhere to say why you changed your name and to what.

Immigration Details Section
Here it asks for other countries she has visited or lived in apart from Thailand and UK. Does this mean every country she has ever visited since birth or from the date she first came to UK with leave to enter?

Non Applying Children
Her son is a British Citizen yet the form asks for details of every country he has visited or lived in. Does this mean since birth or since he came to UK which was 2.6 years ago.

Legally means via deed poll. Not marriage.

This question is not important. Enter simply UK and Thailand
