Medical Conditions and Evidence

Hi all,
We are in the process of applying for ILR(M) for my wife. I receive PIP for chronic conditions.
One of the questions asks, “Do you or your partner have any physical or mental conditions which require at home personal care or medical assistance?”
UKVI are also asking for a hospital Consultant or Specialist Registrar letter as evidence. The most recent letter I have is from a GP Registrar from November 2019 when I was discharged from the hospital clinic. We since manage my symptoms independently.
Please advise, is it okay to answer “No” to this question as there are no social care arrangements etc. or is it better to answer “Yes” and provide the November 2019 letter?
