If logged in for only myself, I saw a few time slots are available. But no appointments are available if logged in for two. I'll be very surprised if all slots are only left for one person. Did someone have similar experience?

Family appointments are harder to get hold of. Keep trying.

Family slots are a bit difficult online but if you look for them daily, you will find them. If unsuccessful, try booking them as single appointment on the same day at the same PEO.
I had no problem booking it in the first attempt but it will depend on the day.
Best regards

finally i found available slots.
in the confirmation page, it only mentioned the details of main applicant. Perhaps i'm bit paranoid, how do I verify the other applicant was booked as well?

If you key in their log in and transaction I'd at the start of the process it'll directly take you to the confirmation page. By this you'll know they are booked too. Make sure you print Both out when you go.
Good luck!

the confirmation page has no name at all after logged back in.
