(Urgent) SET M Application - Married to British Citizen

Hello Everybody,
I am a British Citizen. My wife is applying for ILR using SET M form after spending two years with me in UK. I have few questions and all help would be highly appreciated.
1. My wife booked same day appointment from UKBIA website. There was an option of adding dependents and family members who will be attending the appointment. Does she need to add me to the appointment? or should I just turn up with her on the day of appointment and I will be allowed in because it is SET M application and partner is also required to be present.
2. Do you have to provide Payslips ? or bank statements are sufficient? In my case I am self employed. If payslips are required I can attached my business account statements as well.
3. From Nov 22 fee is going to be increased. There is an option of submitting PDF application form I downloaded from the website and pay it online once we have completed the form. If I do it today? would old fee will apply? Although our appointment is on 3rd of Dec.
4. I am self employed and business is now two years old. Some months are good for money coming in and some are not so good. Would that effect my affordability? we have never used any sort of benefits what so ever.
5. We dont receive money from anybody on regular basis. but In January 2010 I received 55k from my father Abroad as a gift to pay for the deposit of house we own. Should I show the letter of transfer? Would this help that we own a house and we have a good stake in it as we paid a very high deposit.
6. We have plenty of documents at the same address. but nothing in joint names. Only thing which shows jointly is our phone bill. Account is in her name and It comes from my direct debit. Do all documents have to be in joint names for proof of address?
