ILR SET (O) FORM QUERY FOR Pre Nov 2006 HSMP

Hi
I got my initial visa in august 2006 (HSMP) and extention in 2008 till august 2011 (stamped on passport as tier 1 general but letter from home office says extention on the basis of high court judgement on 8 april 2008 regards to the changes in HSMP in december 2006).
Please help with two queries
1) in the form what do i tick
a)tier 1 general applyin under HSMP forum judicial review policy
or
b) Hily skilled migrant under the terms of HSMP ILR Judicial policy
2) in section 10 which do i follow
10 G or 10 H
