ILR due, list of documents, please cross-check

Hello All,
This forum rocks and special thanks to all the forumites for making this forum what it is today...
I've my ILR due in 20 days but due to some personal reasons I am planning to apply after a month...Below is what I think is the list of documents I think i should be carrying along , would really really appreciate if it can be cross- checked by fellow members. Your comments are most welcom.
5 years P60
Personal Bank statements (some of them are on formal printouts and some months I'll get prinouts from HSBC)
Quaterly payslips issued by my accountant (I hope this would not be issue)
Dividend Vouchers + Minutes of meeting
Business Bank statements
Accountant letter confirming the salary and dividends I've received so far
Life In UK test certificate
Work Contracts
Set(O) form
Photographs
Absence sheet
Would really appreciate if some one can check and let me know if I'm missing anything in the list, also would be grateful if someone can post a template for the accoutant letter.

You should indicate what category you are applying under? because documents requirement varies depending on which type your case is.

I am applying under 5 years rule for Tier 1.

Make sure that the Bank statement printouts are either original on the bank letterhead, or stamped and signed by the bank otherwise they won't be accepted. Rest seems fine.

Hi,
I am also going to apply for ILR under (WP + Tier-1 General)
Can you kindly share the template you used for the following:
Dividend Vouchers + Minutes of meeting
Accountant letter confirming the salary and dividends I've received so far
I am on Tier-1 as my current status, do you think i need to arrange for employer's letter for my previous tenure as WP.
My ILR application is due in september so the template and info will be very useful. for me as well.

Thanks! Do you think the work contracts and business bank statements are required to be produced?
Reason for this question is, in tier one tenure I have used a payrol company to run my payroll and in another contract I started my own limited company.
Also if you have any format for dividends voucher , minutes of meetings and letter from the accountant, that will be very useful.

Thanks xyz, really appreciate... i have nearly 120 days of leave...so I think am pretty much on the safer side.
Do you know if you have any template of letter i need to get from accountant which would mention all my earnings in different categories like dividend + salary..etc
