Specific help - Confused ILR due

My Visa was stamped first on 23rd Oct 2006 as HSMP. My day of arrival in UK was 08th Dec 2006 46 days after the Visa was stamped.

Extension applied and granted until 05 Nov 2011.

As per 28 days rule this is still short by 5 odd days.

Please answer and help. Be specific I don't understand too much of policy. Will I have to apply for extension and then ILR .

Please please specify. Also what section will I mark on the Form in section 3 SET(O).

If we are referring to leave granted then extended in the same category for a period of five years, you may include the time spent between the visa emission and your arrival in the UK into the calculation of the residence period.

Yes I came on HSMP and was extended as Resident Visa for 3 years so 2 + 3 years, so I can I count it as 5 years?

Also what I should mark in section 3 in SET (O) Form

What will I mark in the section 3 of SET(O) form as HSMP Judicial Review or HSMP Forum review..Please note that I was sent an approval letter dated 13.06.2006 and I got VISA on 23rd Oct 2006.
