SET(O) form querry

Dear Sir/ Madam,
I will be applying for ILR at the end of November 2011 with a combination of 4 months of work permit, 2 years of initial HSMP and 2.5 years of being under tier 1 general visa.
Work Permit granted 10th December 2006
HSMP granted 09th February 2007
(Shift to) Granted Tier 1 on 09th March 2009.
I have a few querries.
1. What is the earliest that I can apply?
2. When completing form SET(O) - Section 3, I am unsure whether I complete Section A or Section B as both mention Tier 1 . Also confused because there is no mention of people applying with a combination of WP + HSMP + Tier 1 (General) in either section.
3. I plan to apply in person. Can my wife apply with me on the same day using FLR(M) as she is currently on Tier 2 (General)
