SET(0) form question 6.1 query

Hi everyone,
I got HSMP approved in october 2006, got HSMP visa stamped in december 2006. I came to the UK as a student in 2004.
question:
6.1 of the application form do i enter the date I first entered UK like 2004 in my case or the date i shifted to HSMP?
6.2 list the absence do i need to enter right from the day i entered the UK or from the date i shifted to HSMP
