Questions regarding filling up SET(O) form

Hi All,
I am new to this forum and need your help with some basic questions: I am applying for my ILR in Jan 2012.
1. I have downloaded the SET(O) form from the UKBA website. When i print this form can i print this form back to back on a A4 page or should each page be printed on a separate page?
2. After marriage I have not changed my name on my passport however I have the following items under my married name( my name is same but my surname has changed after the marriage):
a. Pay Slips
b. Bank account
c . Utility Bills
So should I fill the 1.6 section on the SET(O) form? Under section 1.6 "Any other name(s) by which you are or have been known" ?
I have following documents in support of point 2 above:
a. Letter from the Bank confirming that the same accountholder was in the past known with maiden name
b. Stamp on my passport from Indian embasay in Edinburgh to confirm my spouse name on the passport. So I have a stamp in my passport showing my husbands name and a stamp on my husband's passport showing my name as his spouse.
c. Marrriage certificate

I am the main applicant and applying from Tier 1 to ILR

1. Yes u shud as u have a different name before marriage and now it is changed.
2. Wen i applied for my visa i put it in my form and had the visa issued without any problems.
