Need some guidance being confused!!

In passport, Surname & Given Names mentioned as below,

Surname: Jitendra
Name: Kapoor

Correted this sometimes back via deed here and now all bills, payslips,
banks have got "Jitendra Kapoor" but haven't changed the passport and
visa since this is a long complex process,

Question is, what to fill in 1.4, 1.5 & 1.6 now?

To go as per passport in 1.4 e.g. "Kapoort Jitendra" and then 1.5 as "Jitendra" also 1.6 as "Jitendra Kapoor" OR

To go as per new name but then this will differ against PP & existing visas.

1. Go with wat it says in u passport.
2. There is a section in the form where it says " Have u been known with any other name" in this section state the name change and provide the evidence to support this.
3. The process is long but u shud have done it but i do not see any serious concerns as long as ample evidence is provided to support this.
