Hello dear, as long as ur salary is concerned it should meet the ukba's code of practices, you can check o n their website that which soc code an assistant restaurant manager comes and accordingly it will show the salary requirements. For the lost copy of work permit you can attach a cover letter with your application and explain that why you havent got ur work permit sheet or best seek some solicitors heo

1. make sure your salary is calculated correctly - your employer should use the same format as in SOC document (http://www.ukba.homeoffice.gov.uk/sitec ... iew=Binary) (e.g. rate per hour or annual salary; based on how many hours per week...) when stating your salary in the letter.

2. your letter explaining why you don't have WP can be supported with the WP-based Visa stamp in your passport - WP number should be on that Visa sticker
