ILR-Forms

I have few questions regarding the SET(O) forms, can anyone advise me on this.
About my details, I came to UK on student visa in 2003 for Phd and started working in a same company after I finished Phd on student visa and later company applied work permit for me 2007.

Questions are:

1. Section 1.9, Passport details, last month I renewed my passport, which passport details I should mention, one which has Work Permit stamp or the new passport.
2. Section 2, Child’s Nationality, my child is born in London few months back and we have not got his Indian passport so what I should write about his nationality and do we have to mention him as ILR dependant.
3. Section 6.1, Main Entry, although I came to UK in 2003 but got work permit in 2007, so what I should write in the section 6.1
4. I got married 1.5 years back and my wife has not completed her 2 years, does she needs to apply FLR form or this rule exempt for old style work permit (Before PBS).

New Passport

If you have no plans to travel you can apply for his/her British passport soon after your ILR is granted.
Alternately, get Indian passport for Child and apply as your dependent

3. Section 6.1, Main Entry, although I came to UK in 2003 but got work permit in 2007, so what I should write in the section 6.1

She will have to apply for FLR.

1. Yes new passport.
2. Regarding Child Nationality you do nt need mention leave it as blank-empty once you get ILR you need register him for British Citizen.
3. Regarding your entry date, you need to mention your entry clearance date not work permit.
4. I do nt think you need to apply FLR form for your wife as you are on old style work permit, which has exempted this rule.

Senior Clarify me, if i am wrong.

Thanks all for the replies, there have been mixed replies can anyone please advise me whether to write my son details on the form or not, he is born in England last month and also i am on company sponsored work permit- (before PBS) and my wife has not completed her two years of stay in UK, is it she eligible for ILR at same time with me or not?
