Non EEA family member to EEA citizen - PR application

Dear Forum,

Can you please advise which is the correct form on the Gov.UK website, is it what is called EEA (PR) version 01/2015? A 121 page application form?

I have a question, my partner has been in the UK exercising his treaty rights since 1 June 2009, I received my residence card on the 21 September 2016, I have had two UK entry permits (visa EEA FP: Family Member to acc xxxx) for 6 moths each, first one being valid from 03/08/2010, so I have been here already for 5 years plus? Do apply on his time in UK or mine?
