EEA FP FOR spouse of 5 YEAR RECSIDENCE CARD holder

Dear members ,

hope you guys can help me,

Actually I am 25 family member(daughter) of eu national, I am currently holding 5 years RC . I got cause I was dependent on him and have been living with him since I got RC.

I recently got married to non eu national I want to apply his EEA fAMILY PERMIT. I checked on UKBA site about family permit it says

You can apply as an ‘extended’ family member’, eg brother, sister, aunt, uncle, cousin, nephew or niece.

You must be able to show that you’re dependent on the EEA citizen or a member of their household,

I am self employed since 2 years. I want to know how can I apply his visa ?

what documents do I need to submit ?

Shall I submit my documents or my father ? OR both of us?

please guys help will be much appreciated.
