EEA1 and EEA2 new application how?

Dear all,

My wife(EEA) and I(NON-EEA/EU) want to submit application for EEA1 and EEA2. Do we have to use one form(https://www.gov.uk/government/uploads/s ... _07-15.pdf) or we have to fill in the forms separately? I read from this forum that we can send our application together and in that case, I am not sure if we can use a single form for us both or separate forms.

Lastly, aside from my wife's play slips and letter from her employer, is it mandatory to include tenancy agreement ?
