Its in the UKBA website

If you are a parent or guardian of a child under 11 years of age you need to bring them to the clinic and complete a health questionnaire for them. The clinician will then decide whether they need to be screened. If the clinician decides not to screen your child, a certificate will be provided to state that your child was not tested. This certificate must be submitted with your child's UK visa application.

Is the clinician present in the VFS centre OR do we need to get this health questionnaire get completed by visiting the approved clinic.

Thanks
