From witch country you are going to apply?
Your spouse is with you? You are going to travel together?

here is a slightly edited version of what we used:
Dear Sir or Madam
Re: EEA Family Permit application GWF123456
I am now applying under EU Directive 2004/38/EC as the family member of an EEA national.
I enclose the following documents to demonstrate that we qualify for a EEA Family Permit.
• Evidence of EEA national being a worker and that the my spouse is therefore a qualified person: Employer P35 return / letter from employer / HMRC tax calculation
• Evidence that we are married: Original Marriage Certificate (and fiancé visa verifies not marriage of convenience)
• Certified copy of Husband’s EEA passport / My original passport
• Letter from my husband confirming that I am joining him.
Thank you for your help
Yours faithfully
