How to submit another EEA2 application?

Hi all,
Our EEA2 application was rejected for the reason of not providing sufficient evidence for comprehensive health insurance whilst being a student. The non-EEA passport was retained. We will be submitting another application but now as a employed individual. I was just wondering;
1. Do we just use the same EEA2 application form as we did the first time?
2. What about the non-EEA passport that was retained?
3. Shall we send in a copy of our rejection letter (and the old Confirmation of Application)?
4. Do we know whether the introduction of a £55 fee has improved the overall processing time of the EEA applications?
Any tips are much appreciated.
