EEA family permit for non EEA parents.

HI ALL,
First of all I'd like to thank you all you guys for helping so many people as my self & all the advice, & help you all have given me in the past. Hope you will do the same again.
So in brief : I'm a Sri Lankan, married to my polish wife & we have a 6 months old child. Both working part time while claiming , housing, child & working tax credit benefits.
My parents live in Sri Lanka, both in their 70's & in poor health (heart patient, poor vision & some minor issues) so me & my wife were planing to bring them here to live with us in UK for both their & our benefit.
So my questions are :
1. Where should I apply from (UK or SL)& how?
2. What documents should I provide?
3. Would claiming benefits be an issue?
4. Would it be enough if I provide last 5 months receipt of money transfers?
Please help me with this matter & possibly with some internet links if you could.
