EEA4 PR for Dependant monther

Hi
What can i write in my covering letter to prove that my mother is depended on me? I have GP letter stating she has long tream health conditions (Type 2 Diabetes/High BP) and she regularly visiting surgery with us. I am plaining to include GP letter and some NHS hospital letters. what else I should include or write in the covering letter to prove the dependency?
Also proof of residence. Our Joint Bank statements for last five years is sufficient or i have to also include tenancy agreements, utility bills, etc. I don't have tenancy or utility bills because i live in my bother flat.
