My residence card expire in 2015.
Pls what form will i apply form eea4
or eea3?

And what residence permit will i be granted?

Once you complete 5 years of residence, you can apply for Confirmation of PR status using form EEA4 providing evidence of your ex treaty rights for the period up to to the divorce and your treaty rights evidence from the divorce onwards.
