EEA Family Permits

Where can I get an application for family visa? Iam visiting Scotland with my Spouse, EU passport holder and Iam Zambian both living in Ireland. I believe I can't travel with him without a visa, but require a family visa and it's free. Can someone pls send the link for this application form? Thanks

What is your EU citizen spouse's citizenship?
Do you have an Irish issued 4EUFam (also called a Residence Card)?

I am a EEA national.
My mother and sister are Indian. Can they apply for Family permit to come to UK for holidays?
Or they need standard visa?

A British citizen applying EEA family permit (UK) for his non-EEA family. What selection should I make for the following while making online application.
