EEA PR and WRS after 10 Years

Hi,

I have tried to find the answer on the Internet on this forum but unfortunately it seems that such situation is rather rear.

I am in a process of applying for the Permanent Residency using a new EEA(PR) application form but I am bit confused about dates to put.

I am Polish citizen who have been living and working (no break in employment apart of holidays which is normally 27 days a year) in the UK for almost 11 years. I arrived here in 2004 and I registered with WRS. I worked for that employer for 16 months, so I had completed the 12 months period. Then I worked for another 2 employers and I am still working with one of them.

On the EEA PR I am asked to put 5 years during which I was exercising treaty rights. My Question is:

Which dates do I need to put? Do I put last five years and Include documents for WRS?

Also there is a question 5 Section A to list all activities "since you've entered uk". Do I have to list all my jobs here? Or last 5 years?

Last 5 years and you will need to provide WRS as well.
