EEA Family Permit application on Visa4uk SO MANY QUESTIONS

Hi Everybody!
I'm applying for the EEA family permit (I am Canadian and my Husband is Dutch) and so far I have a few questions regarding the Visa4uk application. I've been searching these message boards and have not found any specific answers for my questions so I hope you can help!
1- I have lived in the UK for the past 5 years under 3 different visas (Tier 4 student visa, tier 1 post study-work visa and most recently tier 4 youth-mobility scheme visa) and I need to go back to Canada to apply for the EEA FP (and the terms of my current visa make it clear I must leave the country to apply for a new visa). For the section where I am meant to list my permanent address should I list my most current address with my Husband in the UK or my parents address in Canada where I will be staying while I apply for my visa?
2- Under the section "EEA National" they ask for information such as your Family Name / Surname, DOB, Passport info etc., but just below in the same section they ask questions like when did you last see the EEA National? Am I meant to be filling in my details (again) or am I meant to be filling in his? As they do not specify at the beginning this is very confusing.
3- Medical information. I've been living here for 5 years so I have popped in to the doctor when I have been ill occasionally but I've not been admitted to Hospital or had any surgery or anything. Is it necessary that I fill this section in?
4- in the section Employment, Income and Sponsor Details I am filling in all the info but my understanding is that this has nothing to do with their decision on whether or not my visa is accepted. Is this correct?
5- Also they ask multiple times if I will be travelling with my EEA partner- yes or no. He will already be in the UK so I will be joining him. As this is the case should I put no?
If anyone knows the answers or has been through the application please let me know what's up. Thanks so much!
