Few basic questions about EEA/PR form

Greetings, I am going to apply for PR card soon and have already started doing my homework in terms of collecting all the papers and checking the latest form.

I would like to ask few questions about EEA/PR as some areas of this document are not very clear and I am struggling to understand what exactly is required. Any help or guidance is highly appreciated.

One of the questions I have is related to 5.5.

Being a passport holder/citizen of EU country i have moved to the UK 5 years ago. 2 years later after the move i have applied for registration certificate EEA (not sure about the right term for it) from home office.

In the home country i am still considered as citizen/non-resident as i am not spending more than 180 days there. I don't have any additional documents except my passport and ID card.

QUESTION.
What documents must be provided here?
Should i include my registration certificate issued 2 years after i moved?




Second question.
Proof of residence in UK (sections 4-8) or EEA state (section 10) for the relevant period –
e.g. utility bills, bank statements, council tax (or equivalent) bills, NHS (or equivalent) letters,
mortgage/tenancy agreements, etc.


Is it sufficient to provide annual bank statement and annually/quarterly issued utility bill from EDF? Or should i provide a pile of monthly statements covering the 5 years period?


Similar question related to evidence of employment.
Would that be sufficient to provide 4 P60 forms for 5 years or should I provide monthly payslips covering the entire 5 years period?

9B. Accession state workers
Complete this subsection if you are/were, or your sponsor is/was, a national of one of the
countries listed below (‘accession states’), and you or they were working for an employer during
the relevant ‘accession period’ (defined below). You must show that you/they were registered
or authorised to do the work in question, unless you/they were exempt from that requirement. If
you/they were not exempt, you/they must have worked for an uninterrupted period of 12 months
in accordance with a relevant registration or authorisation document to become exempt.
EU8 nationals

Accession period: 1 May 2004 to 30 April 2011
Relevant scheme: Worker registration scheme (WRS)
Relevant authorisation

I have started my employment (treaty rights) 2 weeks before 30th of April and did not apply / register for WRS as I was advised by Home Office not to do it as it was few days before the regulation took place.

My idea is to provide the evidence of my employment starting from 1st of May 2011 (after 30 April 2011) avoiding additional complexity throughout the application process.

The only concern i have is that i don't have my payslips for April, May, June. I only have my payslips from July 2011 and there is no way i can retrieve the missing ones now.
I do have P60 for 2011-2012 however it doesn't show the day I started my employment.

QUESTION
How to prove that i've been treating rights starting from May 2011.

I should be able to get bank statement for May - December 2011 showing that I've been getting salary during this period of time.
Additionally i should be able to provide my job contract stating that i should start my employment the latest by 1st of May 2011.

Would that be sufficient?
