EEA (FM) Certificate of Application ( CoA)

Dear Friends,
i would like to ask you how long it takes to receive CoA from the time of Application.
My time line :-
application sent/Receive : 15.12.2015 - 16.12.2015
Bio- metric Enrollment : 08.01.2016 (dated) 13.01.2016 (received)
Enrollment done : 15.01.2016
waiting CoA :
awaiting to hear from your experience and reply !!!

After they receive your biometrics, you should receive your COA within 21 working days.

I receive my COA letter with right of work today.
Application sent: 10.12.2015
Received by HO: 11.12.2015
Payment Taken: 15.12.15
Biometric Letter Rec: (dated: 15/01/16)
Biometrics : 15/01/2016
COA with right to work: 29/01/2016 (Dated: 28/01/2016)
Received Quetionnaire:
RC & RC: ?

May I know what's this questionnaire that you received with COA ?
And what is RC & RC ?

I assume it would be Registration Certificate and Residence card

I have received my CoA (with Right to work)- 01/02/2016 (dated 28/01/2016)

sorry for late reply as I was busy in shifting my house. this questionnaire thing i found it in one of the forum guy who received questioner about his marriage life and history of relationship. not every one get this some of them got this so thats why I mention this in my message. nothing is serious.
