EEA3/EEA4 application issues, help needed

Hi there,

I am a Polish national and my husband is Colombian. We got married in May 2010 and applied for a Residence Cards which were granted. We have applied for the EEA (PR) for my husband in 2015 and were refused due to not enough support documents and inconsistencies in the application.
We are now trying to apply again and I have some doubts on how to proceed.

Can we apply using the same form (me as the main applicant for a permanent residence and him as a family member) or should we do it on two separate forms?
We got married in May 2015 and I was excercising my treaty rights (except for 1 month in 2011 when I was looking for a new job, didn't have CSI then) until June 2015 when I left UK for 5 months (went travelling and spent some time with my family in Poland). I came back in November 2015 and started working again. from June until now I have been away from UK for 5 months and 3 weeks but now I am going again in two weeks for two weeks to Poland? I am now scared that this may compromise the application
Which periods should I use in my application? May 10 - May 2015 or last 5 years? If I choose the first option should I also include documentation for the last year and disclose why was away - maybe in the cover letter?

Unsure on how to proceed and quite stresses frankly. I would really appreciate any comments on this.

Any 5 year period for which you have good (if not unimpeachable) documentary supporting evidence can be used.
Make sure you address the irregularities and flaws from your first application.

As an A8 national, were you fully and properly registered for WRS up until 2011?

You can both apply on same form.

You are allowed time off from UK for good behaviour;
so an absence of up to 6 months in a 12 month period (running yea-on-year from your wedding anniversary) should not be an issue for continuity of residence.
You don't have to justify it but can if you wish.
